We aim to make the process as simple as possible.

To refer a patient to our Inpatient Unit or to the Hospice at Home service please complete the referral form and return to [email protected]

Only referrals made by health care professionals will be accepted for these services due to the level of clinical information required. The referral will be triaged and discussed with you and if appropriate an admission planned.

Other Services

To refer to the Tulip Centre or Family and Carer Support Services please complete the referral form and return to [email protected]. We accept both professionals and self-referrals.

For self-referrals call 01485 601700 Mon-Fri 9-5. We will take your details and someone from the team will call you to complete a referral form with you. We may also contact any health professionals involved for further information.

Please be aware that we are unable to take direct referrals for Social Work only input at present due to this role being vacant. For Social Work referrals please contact the SCCE team at Norfolk County Council on 0344 800 8020. Thank you.

The referral will be triaged and discussed at our daily referral meeting (Monday - Friday), where it will be passed o the appropriate team to make contact.

If you need to discuss any of the above please ring 01485 601700 and ask to be put through to the ward clerk where you will be able to leave brief details and the most appropriate person will call you back.
