How we help Family and carer support Patient and family support The Hospice social worker, chaplain, counsellors and trained volunteers provide support to patients and families during times of illness and following a bereavement. Support can include talking about the future, managing important conversations with people close to you and helping you with adjustment to health changes. Home visits may be available as well as appointments at the Hospice. Our Specialist Palliative Care Social Worker can help with a wide range of issues, including financial advice, emotional support and advanced care planning. Funded by both the Hospice and Macmillan Cancer Support, the social worker offers advice and advocacy to anyone with a life-shortening illness in West Norfolk. To make a referral for social work support, please call the Co-ordination Centre on 01553 668526 or call the Hospice directly on 01485 601700 to speak to our social worker.
